  • Just in case of problems with e-mail communications, we ask that you share your phone number with us.
  • Please fill in the name of the workshop tour or private tour through which you're traveling (e.g., Tropical Bird Photography with Greg Basco and Doug Brown or Bob Smith family private tour).
  • Please provide the emergency contact information for someone not traveling with you to Costa Rica (name, e-mail address(es), home phone, cell phone).
  • Please provide your complete flight information in a clearly decipherable manner. Please include carrier, flight #, dates and times, including any connecting flights, for your arrival to and departure from Costa Rica.
  • Please describe any diet preferences or restrictions that you might have. If left blank, we will assume that you have no dietary issues of which we should be aware.
  • Please inform us of any health issues you feel to be relevant to your Costa Rica travel, including any medication allergies. If left blank, we will assume that you have no health issues of which we should be aware.
